An avoidable accident

NTSB Docket on Phenom crash=

Although this was a VLJ, this is an example of what happens when systems-related complacency sets in for any single pilot IFR operation. This pilot was not vocalising checklists, was joking with passengers when flying an instrument approach in IMC and unaware of his aircraft state. It seems likely the aircraft accumulated ice through operating in IMC below 0° without anti-icing systems activated and was not monitoring his airspeed.

The aircraft then stalled and departed at low altitude. Frightening.

It would be interesting to see accident statistics on pilots flying privately versus number of hours. It seems that complacency seems to be a big problem for high-time pilots that aren’t subject to the stricter controls/checks.

It would be hard to get good data though as many high-time pilots, such as the one in this crash, hold CPL, FI ratings etc. but probably still only fly privately.

Accidents are always a chain of events usually pointing to pilot errors. There are probably low time pilots who are already getting into a bad habit of taking shortcuts. And likewise there are high time pilots doing the same thing. The key is to not be in a plane with someone who pilots like that…

During training, I have on two occasions become distracted by the chit-chat of instructors and had WTF moments. Instructors should know better. So now I make a point of no chit-chat during terminal and approach phases.
