Poor ergonomics of phones. The only way to hold them in one hand, without way too much finger gymnastics, and at the same time take photos or videos, is to hold them in portrait mode.
I read about this one. „Target fixation“. Error of judgement.
Imagine if the drop operator had not released the extinguishing agent… they’d have hit the ridge.

Probably fairly easy to do; I’ve gone around on a TCAS warning of somebody below me who was radio silent (or misreported position). One can’t see below oneself.

My last Skydive down in the West Country the year before last involved a day with a surprising amount of traffic and I can only thank the stars that on the whole these events arent more common, it is a strange feeling with aircraft close, having no idea what they might do next, and hoping they arent going to head in your direction.
