Add to that France where it is also an area forecast and a reference altitude (see https://aviation.meteo.fr/gafor_expair.php). I am not sure how the reference altitude is calculated for France though.

Your understanding agrees with mine, but it is not compatible to what chrisn and jgmusic writes — that the reference altitude is the minimum altitude along a route.
