EASA survey on ADS-B and collision avoidance in VFR aircraft

That site gives me an error “Advanced blocking in effect” (looks like some kind of rate limiting scheme).

Not that I don’t welcome any effort to increase safety but this survey appears a bit amateuristic to me:

A large part of the requested data is available through each memberstate’s aircraft register
Nothing about this survey can be found on the EASA website, so no source data available
Using an Excel-sheet that needs to be emailed is quite user unfriendly. Using Google Forms is quicker and easier
Stating 2 different deadlines is a bit odd.
